97C WELL FOR LIFE Community Health Hub is a confidential health and wellbeing charity whose aims are to promote the physical and emotional health of people in the city of Derby and Derbyshire County living with long term health condition/s. Gaining a better awareness of their long term condition/s helps people understand their symptoms and experiences and improves their long term health and wellbeing.

WELL FOR LIFE encourages self-confidence and the capacity for self-management, supports people to have more control of their conditions and their lives and promotes their efforts to enhance their health and wellbeing.

WELL FOR LIFE services include, social opportunity, ‘A place to grow’ counselling service, help with form filling, benefits advice, advocacy, support with DWP medicals and work capability assessments, health, lifestyle and dietary advice and sexual and general health monitoring and screening.

WELL FOR LIFE services are open to both adults and young persons over age 16 and services are offered irrespective of gender, sexual orientation, ethnic or cultural background, to people of any faith, or none.

WELL FOR LIFE also provides free general and sexual health checks including, blood pressure, blood glucose, cholesterol, BMI, rapid HIV tests and sexual health screening.
